Safety standards are written in response to incidents. Reading them as a compliance checklist misses the point — each clause exists because something went wrong somewhere.
Machine guarding
The most common gap we find is guarding that was correct at installation and has since been defeated for convenience. Design guarding that does not slow the operator down, and it stays in place.
Lockout / tagout
Procedures are usually adequate on paper. Enforcement during a rushed changeover is where they fail.
